Our company consists of three related business lines. Therapy Management, Inc. (AKA “TMI”) provides physical, occupational, and speech therapy to residents in Skilled Nursing Facilities. EverActive Therapy establishes and manages therapy clinics in assisted and independent living communities. EverHome Skilled Home Health Care & Personal care provides nursing and therapy (PT, OT, ST) in the home setting. Here are details about each company:
Therapy Management, Inc.
Since 2007, we’ve been creating therapy programs that uniquely mesh with the goals and culture of each senior community we serve. Clients describe us with words like – creative, flexible, joyful, dedicated, industry experts, hands-on, and willing to go the extra mile.
Our services include – physical therapy, occupational therapy, speech language pathology rehabilitation, and wellness programming.
Our first core value is “Do what is right for the person.” Patient/resident care is our first priority, and our expert clinicians always do what is right for those who trust in us. We are therapist-owned and operated. Our home offices are located in Michigan.

Skilled Home Health Care and Personal Care
Since 2004, EverHome (previously Diamond Home Health Care) has provided skilled home care including nursing, therapy (physical, occupational, speech) and private duty home care (personal care, companionship, and support).
EverHome has two offices – one in Plymouth and one in Bay City, Michigan. We are a Medicare and CHAP certified home health agency servicing areas across Michigan – including southeast Michigan, the Detroit suburbs, Ann Arbor, Ypsilanti, South Lyon, Brighton, Saginaw, Bay City, Midland, and surrounding areas.
We are privately held, owned, and operated by health care professionals. Our teams specialize in orthopedic conditions, Alzheimer’s and dementia care, neurological rehabilitation, and care for those with a wide range of diseases and conditions. In addition to our skilled clinical services, we provide personal care and support services at home. Our caregivers are warmhearted, generous, and do whatever it takes to help people live safely and securely in their familiar home setting where they are surrounded by family, memories, and often their loving pets.
EverActive
Therapy Onsite Therapy and Wellness
EverActive Therapy provides onsite physical, occupational, and speech therapy to residents at King’s Daughters Home. Our therapy and wellness programs are specifically designed to enable older adults to stay active, improve balance, maintain & gain strength, manage ongoing conditions, and age optimally.
PHYSICAL THERAPY
Physical therapy helps restore and maintain functional mobility, strength, balance, and endurance, while alleviating pain.
OCCUPATIONAL THERAPY
Occupational therapy empowers residents to improve daily skills like dressing, eating, hygiene, and home management.
SPEECH THERAPY
Speech therapy improves and/or preserves cognitive abilities, promotes speech and language, and treats swallowing issues.
RESILIENCE EXERCISE PROGRAMMING
With “Resilience,” each resident receives a personalized exercise program developed and overseen by their therapist.
WELLNESS PROGRAMMING
Our teams conduct community classes like cardio drumming, balance clinics, chair yoga, and more.
