How does insurance work?
Residents can receive therapy (covered by insurance) after discharge from a home health episode or as a separate course of treatment as prescribed by a provider (physician, nurse practitioner, physician’s assistant).
What happens before, during, and after outpatient therapy?

First, your doctor, NP or PA writes an order or a referral for outpatient therapy. A skilled therapist then conducts a comprehensive evaluation, which is used to create a plan of care (POC) based on your medical history, standardized assessments, your therapists’ clinical observations, and your goals.

Our therapy team then works with you to complete your plan of care. The frequency of your treatment plan depends on your POC, capabilities, and progress toward goals.

When goals have been achieved and when skilled care is no longer required, then your therapy team will provide you with the resources you need to continue your health journey. This could include an ongoing exercise plan, planning for your daily routine, and training for you, your family, and your caregivers.
